School Based Health APP - Job Overview

Schedule: Monday-Thursday (7AM-4:15PM) | Friday (7AM-3PM)

Join our Legacy Community Health family as a Nurse Practitioner or Physician Assistant in our School-Based Health Program! In this vital role, you will administer compassionate care to children within the school clinic setting and assist the family members of enrolled students—all under the supportive supervision of our physicians.

Key Responsibilities

  • Administer routine pediatric care in school-based clinics, including medical procedures and drug treatments.
  • Follow the Nurse Practice Act and maintain compliance with policies and protocols.
  • Assist in assessing client services and report needs for improvement.
  • Participate in clinic development and implementation alongside our health team.
  • Document all client care accurately and promptly.
  • Consult and triage clients effectively, ensuring they receive the care they need.
  • Evaluate lab results and follow up on abnormal values in collaboration with the supervising physician.
  • Engage in patient education about health maintenance and disease prevention.
  • Participate in interdisciplinary team meetings and performance improvement programs.
  • Demonstrate respect and professionalism toward all patients and staff, contributing to a positive work environment.

Minimum Qualifications

  • PA: Current Texas state licensure or eligibility for licensure and graduation from an accredited program as Physician Assistant
  • NP: Current license from the Texas Board of Nursing and completion of program at an accredited school.
  • Provider shall obtain, maintain and participate in the Medicare and Medicaid Programs, workers’ compensation, other federal and state reimbursement programs, and the payment plan of any commercial insurer, health maintenance organization, preferred provider organization, accountable health plan, and other health benefit program requested by Legacy.
  • PA: 1 year of medical experience in a physician’s office or equivalent combination of training and experience
  • PA and NP: 2-5 years experience is preferred.
  • PA and NP: School-based (SBH) clinic experience is highly desired.
  • Must be able to use electronic medical record with proficiency. 
  • Ideal candidate is independent, enthusiastic and highly motivated.
  • Must be able to manage multiple deadlines and prioritize.
  • Bilingual in Spanish is a plus.
